Current driver’s abstract
A current driver’s abstract that can be obtained at any Illinois Secretary of State office.
Statutory Summary of Suspension
The “Statutory Summary of Suspension” that was issued by the arresting officer at the time of your arrest (it’s a white form that has your BAC results, or “checked” if you refused chemical testing).
